POSITION: Bilingual LVN Nurse Health Educator
SCHEDULE: Monday through Friday and PRN
RESPONSIBLE TO: Director, Adult Health Center
SALARY RANGE: Full Time
SUMMARY OF DUTIES:
The Nurse Health Educator is responsible to the Department Director and the Adult Health Center (AHC) clients for rendering nursing care and creating and implementing health education programs that meet the medical needs of the clients seeking care at the AHC.
DUTIES AND RESPONSIBILITIES:
Health Education:
- Work with the clinic physicians to complete patient education on disease processes, medications, and self-care during clinic appointment time.
- Counsel patients about their health status for the purpose of making educated health treatment decisions.
- Create health education programs for CCA clients and AHC patients.
- Locate community resources that can be used by the patients to assist them in addressing their individual health concerns as well as educate clinic staff about the available services that can be used to complement prescribed patient care.
- Order/locate/create educational materials according to the needs of the clinic’s patient population
- Develop and present health education classes and materials on various health topics.
- Create evaluations to determine the success of health education programs.
- Assess the impact of health education programs and materials.
- Assist the director in acting as a liaison to community organizations affiliated with the health care field.
- Work with the volunteer coordinator in the planning and implementation of annual health fairs presented by the AHC. Duties will include the following:
- Contacting organizations
- Purchasing supplies
- Soliciting donations
- Record keeping
Nursing:
- Lead staff competency training.
- Assist clinic physicians with “pre-appointment” screenings. Prior to appointment time ask patient about medications; i.e. how they are taking medications, dosage, side affects etc., to ascertain if medications are being taken correctly and address any questions that the patient has about their medical condition.
- Provides clinical support, i.e. – patient assessments, monitors vital signs, administers IM and oral medications, and assists providers during medical procedures.
Grants:
- Assist CCA's Development Analyst with RFPs that have been identified as potential sources of funding for the AHC program by locating required documentation from the AHC's electronic database.
Other duties:
- Assist office staff with front desk as needed
- Checking in patients
- Answering phones
- Pulling patient records
- Precepting Medical Assistant Externs
- Additional duties as assigned.
QUALIFICATIONS:
- Active and unrestricted nursing license in the state of Texas.
- Active BLS certification
- Fluency in Spanish
- Minimum 1 year experience in patient education required.
- Proficiency with Microsoft Office applications such as MS Word, Excel, PowerPoint
- Proficiency in using audio-visual equipment such as projectors, DVD players, etc.
- Strong communication skills
- Previous experience in grant writing preferred
- Must comfortable with one-on-one and group presentations
- Reports to the Adult Health Center Director.
