Case Manager

Non-exempt - Full Time Position

Responsibilities:
• Handle case management services, including financial assistance, food assistance, and community referrals.
• Complete necessary assessment and needs analysis to develop individualized plans for clients.
• Primary contact and advocate for CCA clients.
• Build relationships with clients and the community to expand CCA's Restorative program.
• Regularly monitor the progress towards goals in regularly scheduled client appointments.

Skills:
• Bachelor’s degree in Social Work or related field, preferred
• Ability to solve problems, make decisions, and resolve conflict
• Ability to fluently speak, read and understand both English and Spanish
• Computer proficiency, particularly in MS Word and MS Excel
• Excellent verbal and written communication skills
